About The Medical Team

The Medical Team offers a broad spectrum of in-home services, including both skilled medical care and supportive personal care:

Skilled Home Healthcare: Skilled Nursing: Provided by Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) for services like medication management, wound care, IV therapy, diabetes management, chronic disease management, post-operative care, and patient education. Physical Therapy (PT): To improve mobility, strength, balance, and reduce pain. Occupational Therapy (OT): To help patients regain or improve their ability to perform Activities of Daily Living (ADLs) and instrumental ADLs (IADLs). Speech-Language Pathology (SLP): For communication disorders, swallowing difficulties, and cognitive retraining. Medical Social Work (MSW): Providing emotional support, connecting patients and families with community resources, and assisting with long-term care planning. Home Health Aide (HHA) Services: Assistance with personal care under the supervision of a nurse or therapist. Private Duty Services: Personal Care: Assistance with bathing, dressing, grooming, hygiene, mobility, and transferring. Companionship: Social interaction, supervision, and emotional support. Homemaker Services: Light housekeeping, meal preparation, laundry, and errands. Live-In Care: Continuous care provided by a caregiver who resides in the patient's home.
